Start with one complete list

Write down prescription medicines, over-the-counter products, supplements, herbal products, medicated teas and alcohol use. Include how often you take them and what time of day. Brand names alone can be confusing, so photographs of labels or ingredient lists are useful.

Sedation can add up

Some sleeping tablets, antihistamines, pain medicines, anxiety medicines and herbal products can all make a person drowsy. Combining them may increase slowed reaction time, confusion, falls or next-day driving impairment. Older adults are particularly vulnerable to these effects.

Melatonin is not the same as a general sleeping pill

Melatonin can be useful for some circadian-timing problems, but dose and timing matter, and more is not automatically better. It can still cause drowsiness and may not address the reason a person wakes repeatedly. If sleep problems are persistent, look for the cause rather than simply layering products.

Do not stop prescription medicines abruptly

Some sleep, anxiety or mental-health medicines need gradual changes under medical supervision. Sudden stopping can cause rebound insomnia or other withdrawal effects. A TCM appointment is a good time to review the list, not to replace a medical taper plan.

Alcohol complicates the picture

Alcohol may make someone feel sleepy initially, but it can fragment sleep later and worsen snoring or sleep-disordered breathing. Mixing alcohol with sedating medicines or herbs can also increase impairment. If you are using several sleep aids, mention alcohol honestly to your clinician.

When persistent insomnia needs assessment

Seek help when insomnia lasts for months, causes major daytime impairment, follows a significant mood change, is accompanied by loud snoring or gasping, or requires escalating doses of sedating products. Cognitive behavioural therapy for insomnia and assessment of underlying problems may be more useful than adding another supplement.

How to discuss TCM safely

Tell the TCM practitioner exactly what you are already taking and why. Ask what each herbal product is intended to do, how long it is expected to be used and what side effects should prompt stopping it. If you are pregnant, trying to conceive, elderly or taking several medicines, medication review matters even more.
